1. Acceptance of Terms

By registering for an account, submitting an application, or using the Web-Based Iskolar ng Bayan Scholarship Management System, you agree to comply with and be bound by these Terms and Conditions.

2. Eligibility

Applicants must satisfy the eligibility requirements established by the Municipality of San Rafael and the Iskolar ng Bayan Scholarship Program.

Submission of an application does not guarantee scholarship approval or acceptance.

3. Accuracy of Information

Applicants must provide complete, accurate, and truthful information.

The Municipality reserves the right to verify any information or document submitted through the System.

Any false, misleading, incomplete, altered, or fraudulent information may result in:

• Immediate disqualification from the scholarship program;
• Suspension or termination of the applicant's account;
• Revocation of scholarship benefits if already granted;
• Possible legal action when applicable.

4. Document Submission

Applicants are responsible for ensuring that all required documents are authentic, complete, readable, and submitted within the prescribed deadlines.

The Municipality may request additional documents or clarifications whenever necessary.

5. Account Responsibility

Applicants are responsible for maintaining the confidentiality of their account credentials.

Any activity performed using the applicant's account shall be deemed authorized by the account owner unless proven otherwise.

Applicants must immediately report any unauthorized access or suspected security breach.

6. Application Evaluation

All applications shall undergo evaluation and verification procedures established by the scholarship administrators.

The Municipality reserves the right to approve, reject, defer, or request additional information regarding any application based on program requirements and available scholarship slots.

7. System Availability

While reasonable efforts are made to maintain uninterrupted service, the Municipality does not guarantee continuous availability of the System.

The System may be temporarily unavailable due to maintenance, upgrades, technical issues, or circumstances beyond reasonable control.

8. Intellectual Property

All content, logos, designs, forms, software components, and materials contained within the System are the property of the Municipality of San Rafael or their respective owners and may not be copied, modified, distributed, or reproduced without proper authorization.

9. Limitation of Liability

The Municipality shall not be liable for any loss, damage, delay, or inconvenience arising from:

• Incorrect information provided by applicants;
• Technical interruptions or system failures;
• Delays caused by incomplete submissions;
• Unauthorized access resulting from applicant negligence.

10. Amendments

The Municipality reserves the right to modify, update, or revise this Privacy Policy and Terms and Conditions at any time.

Any significant changes shall be posted through the System and shall take effect upon publication.

11. Governing Law

This Privacy Policy and Terms and Conditions shall be governed by the laws of the Republic of the Philippines, including Republic Act No. 10173 (Data Privacy Act of 2012) and other applicable laws and regulations.

12. Declaration and Consent

By checking the consent box and submitting an application, you certify that:

• All information and documents submitted are true, accurate, and complete;
• You understand that providing false information may result in disqualification or revocation of scholarship benefits;
• You consent to the collection, processing, storage, and use of your personal information for scholarship administration purposes;
• You have read, understood, and agreed to this Privacy Policy and Terms and Conditions.

Privacy Policy

1. Introduction

The Web-Based Iskolar ng Bayan Scholarship Management System ("System") is committed to protecting the privacy and personal information of scholarship applicants. This Privacy Policy explains how the Municipality of San Rafael, Bulacan collects, uses, stores, and protects the information submitted through the System in compliance with Republic Act No. 10173, otherwise known as the Data Privacy Act of 2012.

By creating an account, submitting an application, or using the System, you acknowledge that you have read, understood, and agreed to this Privacy Policy and the Terms and Conditions stated herein.

2. Information We Collect

To process scholarship applications, the System may collect the following information:

Personal Information

• Full Name (Surname, First Name, Middle Name, Name Extension)
• Date of Birth
• Age
• Place of Birth
• Sex
• Civil Status
• Mobile Number
• Email Address
• Permanent Address

Educational Information

• School Name
• Course or Program
• Year Level
• General Weighted Average (GPA/GWA)

Family Background Information

• Solo Parent Status (if applicable)
• Father's Information
• Mother's Information
• Siblings Information
• Parent or Guardian Address, Occupation, and Educational Attainment

Supporting Documents

• Barangay Indigency Certificate
• Request Letter Addressed to the Municipal Mayor
• School Identification Card with Required Signatures
• Other documents that may be required by the scholarship program

3. Purpose of Collection

The information collected shall be used solely for the following purposes:

• Processing and evaluating scholarship applications;
• Verifying applicant eligibility and authenticity of submitted information;
• Communicating application updates, announcements, and scholarship-related notifications;
• Generating reports and statistics for scholarship administration;
• Monitoring, auditing, and improving scholarship services;
• Complying with applicable laws, government regulations, and official requirements.

4. Data Sharing and Disclosure

Personal information shall only be accessed by authorized personnel involved in the administration of the Iskolar ng Bayan Scholarship Program.

Information may be disclosed when:

• Required by law, court order, or government regulation;
• Necessary for auditing, verification, or investigation of scholarship applications;
• Authorized by the applicant through written consent.

The Municipality shall not sell, rent, or disclose applicant information to unauthorized third parties.

5. Data Security

The System implements reasonable administrative, organizational, physical, and technical security measures to protect personal information from unauthorized access, disclosure, alteration, misuse, or loss.

Uploaded documents and applicant records are stored securely and may only be accessed by authorized personnel.

6. Data Retention

Applicant information shall be retained only for as long as necessary to fulfill scholarship administration requirements, legal obligations, auditing purposes, and record-keeping policies of the Municipality of San Rafael.

After the retention period, records may be securely archived or disposed of in accordance with applicable laws and regulations.

7. Rights of Data Subjects

Under the Data Privacy Act of 2012, applicants have the right to:

• Access their personal information;
• Request correction of inaccurate or incomplete information;
• Request the deletion or blocking of information when legally applicable;
• Withdraw consent, subject to legal and administrative limitations;
• File a complaint with the National Privacy Commission if they believe their privacy rights have been violated.
